Service Description

Select this option if you are a new patient or an established patient with a new injury or problem. One of our Doctors of Physical Therapy will travel to you to evaluate you to determine a diagnosis and provide you with high quality one-on-one care to get you back to what you enjoy most. This service is up to 1 hour.

Cancellation Policy

To cancel or reschedule an appointment please contact us at least 24 hours in advance. If you do not contact us at least 24 hours in advance we will have to charge you the full amount for the service, thank you for understanding.
